Petition For Cancellation Form (2011-2012 Academic Year)
The Petition For Cancellation Form must be submitted in person at University Courtyard during business hours (8am to 5pm Monday-Friday). There is a $50.00 fee that also must be submitted with the form in order for the Petition For Cancellation Form to be valid.
